We are looking for 11 men to undergo a vasectomy on July 17 — the Friday of the World Cup Final weekend. Dr. Miller is offering the procedure at half price: $400 total.
Important — New York consent requirement
In New York, a signed authorization form is required at least 30 days before the procedure. If you wish to have the procedure on July 17, your consent form must be signed no later than June 17.
